Instructions: This application must be completed in its entirety. All final or draft advertisement brochures and/or promotional materials if used, must be uploaded. A course syllabus or outline, a vitae of all instructions and (if applicable) a letter verifying the speakers affiliation with an appropriate educational institution must be uploaded.
If you do not wish to upload the above required documents, you must complete the Continuing Education Application and mail in the applicable fees.
If your online course issues multiple certificates per module, you must complete the online application for each module/certificate issued.
All applications must be submitted no less than 30 days prior to the start date of the course.
Completion of this application does not guarantee program approval.
